Ballers Fitness LLC, 140 Eglin Parkway SE, Fort Walton Beach, FL 32548 ("Sponsor"). This sweepstakes is sponsored solely by Ballers Fitness LLC.
NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN. A purchase or payment will not increase your chances of winning.
The Ballers Rewards Program ("Sweepstakes") is open only to legal residents of the State of Florida who are eighteen (18) years of age or older as of the date of entry. Employees, officers, and directors of Sponsor and their immediate family members and household members are not eligible.
Void where prohibited by law. All applicable federal, state, and local laws and regulations apply.
The Sweepstakes consists of three concurrent prize tiers:
Method 1 — Activity Entry (Online): Visit reviews.ballersfitness.net/entries and submit the entry form, listing eligible activities completed during the entry period. Each activity awards a specified number of entries as listed on the entry form. Maximum twenty-five (25) entries per participant per calendar month.
Method 2 — Free Mail-In Entry: Hand-print your full name, address, phone number, email address, and the words "Ballers Rewards Entry" on a 3"x5" card and mail it to: Ballers Fitness LLC, ATTN: Rewards Sweepstakes, 140 Eglin Parkway SE, Fort Walton Beach, FL 32548. Each mail-in entry equals one (1) entry. Limit one (1) mail-in entry per envelope. Mail-in entries must be postmarked by the last day of the entry period and received within seven (7) days of period end.
Entries become the property of Sponsor and will not be returned.
Online entry activities and their entry values are published at reviews.ballersfitness.net/entries and may be updated by Sponsor at any time. Current values include but are not limited to: 5-star Google review (5 entries), friend referral who joins as member (10 entries), social media engagement (1–2 entries), event attendance (5 entries), class attendance milestones (3 entries), and social posts with #BallersNation (2 entries).
Sponsor reserves the right to verify any submitted activity. Sponsor randomly audits approximately 10% of submissions. Submitting false information will result in immediate disqualification for the calendar year and forfeiture of all entries and prizes won.
Entries earned during a calendar month enter that month's Monthly Drawing, that quarter's Quarterly Drawing, and that year's Annual Drawing. Entries do not transfer between participants. Entries reset to zero following each respective drawing.
Winners will be selected by random drawing from all eligible entries received for each prize period. Drawings will be conducted within seven (7) days after the close of each entry period. Odds of winning depend on the total number of eligible entries received.
Approximate retail values ("ARV") and specific prizes may vary by period and will be announced at the start of each entry period. General prize tiers include:
Total ARV of all prizes for the calendar year shall not exceed $5,000. Prizes are non-transferable. No cash equivalent or substitution of prizes is permitted, except by Sponsor due to unavailability, in which case a prize of equal or greater value will be substituted.
Winners will be notified by phone, email, or text message at the contact information provided in their entry within seven (7) days of the drawing. Winners must respond and claim their prize within forty-eight (48) hours of notification. Failure to respond within this period results in forfeiture of the prize, and an alternate winner will be selected.
By accepting a prize, winner grants Sponsor permission to use winner's name, likeness, photograph, voice, and/or biographical information for advertising and promotional purposes without further compensation, except where prohibited by law.
All federal, state, and local taxes on prizes are the sole responsibility of the winner. Winners receiving prizes valued at $600 or more in a calendar year will be required to provide a completed IRS Form W-9 prior to receiving the prize, and will receive an IRS Form 1099 from Sponsor.
By participating, entrants agree to be bound by these Official Rules and the decisions of Sponsor, which are final and binding in all matters. Sponsor reserves the right, in its sole discretion, to disqualify any individual it finds to be tampering with the entry process, violating these Official Rules, acting in an unsportsmanlike manner, or attempting to disrupt the operation of the Sweepstakes.
Sponsor is not responsible for lost, late, illegible, misdirected, garbled, or undelivered entries or for technical, network, electronic, computer, or telephone failures of any kind.
By participating, entrants release and hold harmless Sponsor and its respective officers, directors, employees, and agents from any liability, illness, injury, death, loss, litigation, or damages that may occur, directly or indirectly, from acceptance, possession, or use of any prize.
Personal information collected through this Sweepstakes will be used by Sponsor for purposes of administering the Sweepstakes and for ongoing communications about Ballers Fitness products and services. Participants may opt out of marketing communications at any time. See Sponsor's Privacy Policy at ballersfitness.net/privacy for more information.
This Sweepstakes is in no way sponsored, endorsed, administered by, or associated with Meta Platforms, Inc., Facebook, Instagram, Google LLC, Apple, Inc., or any other social media or technology platform. Participants are providing information to Sponsor and not to any other party.
This Sweepstakes is governed by the laws of the State of Florida, without regard to its conflict-of-law provisions. Any dispute arising out of or related to this Sweepstakes shall be brought exclusively in the state or federal courts located in Okaloosa County, Florida.
For a list of winners, send a self-addressed stamped envelope to: Ballers Fitness LLC, ATTN: Rewards Winners, 140 Eglin Parkway SE, Fort Walton Beach, FL 32548. Requests must be received no later than 90 days after the close of the calendar year.
